Hello I'm Dr. Eugene Corbett from the department of internal medicine here at the University of Virginia I'm here today to participate in a history and physical examination with a standardized patient we haven't met before, but the purpose of this is to provide an opportunity for to you for you to observe how the decision might perform a complete history and physical examination on a patient who's new to them Mr. Murray I'm Dr. Corbin vegetable druggist or night with me too yeah first time we've met yes yeah yes, so I understand you made an appointment for an examination bit of Korea, and we're going to a complete physical examination I understand I'm going to ask you about why you came into that, but we mind if I take a few notes just in case no very close in case you get lost — tell me yeah doing this too much verified but good, so you made the appointment for exam when's the last time I had a physical examination oh probably two years ago yeah well let me tell me why you made this one well I had these chronic headaches going on as I've had for about three months now, and it's just not getting any better and want to see if there's something we can do about it so when you say chronic you're referring to the fact that they've just been steady for three months or even longer than that well it started three months ago they're like every other day or so, and now I'm having them every day and there you have no days when you don't have anything I have a headache every day but at the beginning it didn't seem to be quite as available that poor young Miller just described the headaches where are they and what that what they feel like um they start at the back of my head here and they kind of come up around my temples I guess you call it and into the front of my head sometimes they're dropping you know other times they're not quite as bad, so it's both sides of your you know starts in the back and comes up the front correct do you have a headache all the time or is it just coming um well I have a headache all the time until I take something take Advil and Tylenol that seemed to help it and that eases it up is it ever gone does it always come something there's always something there I always know it's there it's the intensity of it I say can you tell me a little more about what you notice for example when is it the worst at night in the morning, but there's a mega difference um well it doesn't make a lot of difference when I don't get enough sleep it's awful if I move my head in a certain way or if I strain or stretch my head in a certain way, so I went one way can you show me that fuck if I go like you know to the back or if I even am, I'm no bending over you know just any exaggerated moves any exaggerated yes seems to make it worse and if I don't take the Advil in a Tylenol it seems to be worse somewhere — I see so this three months ago it started into both sides it's in the back, and you can make it worse if you either extend or you lean forward for...
